Appointment Information

In order to be admitted for treatment you have to make an appointment. 

Please call (813) 779 8829 to schedule your visit. Please provide information about a reason for your appointment and how would you like to pay for a visit.

We accept majority of the insurances. Our billing team will help you to check your benefits however YOU ARE  responsible to verify the information. 

 If you choose to make a "cash payment" we accept credit cards and checks. 

Our cash Payment rates:

Initial assesment :  $120.00

Treatments : 40 min - $80.00

                   60 min - $100.00

NEW PATIENTS:

2. ARRIVE 15-20 MINUTES EARLY

For first time visit, please arrive 15-20 minutes early to allow for registration and form processing.

3. BRING AN ID CARD

Please bring your driver's license and insurance card with you.

4. WEAR LOOSE CLOTHING

To make your visit pleasant and help you feel relaxed, please wear comfortable clothing.

5 . "YOUR THERAPIST WILL SEE YOU NOW"

Every patient receives an initial evaluation to establish an Individual Treatment Plan. We ask you to tell us about your problems, your expectations from us and from yourself. Then we discuss how we can help you. Together we will establish realistic goals and a time frame to achieve them. The initial visit will take about an hour.

ESTABLISHED PATIENTS:

1. ARRIVE EARLY

Please arrive 5-10 minutes early for your scheduled visit.

2. APPOINTMENT CANCELLATION

If you cannot keep your appointment, kindly let us know a day (24h) in advance . Please call (813) 779 8829. Please refer to our cancellation policy. Cancellation fee may apply.
